The terms that apply to this website, to products purchased from us, and to text messages we send.
Using this site
This website is provided so you can learn about our clinic and request services. You agree to use it only for lawful purposes and in a way that does not interfere with the site or others’ use of it. You may not attempt to access restricted areas, scrape or copy site content in bulk, or submit false information. Submitting a form or request through this site does not by itself create a clinician-patient relationship or confirm an appointment — our staff will contact you to confirm.
Not medical advice
The information on this website is general and educational. It is not medical advice, it does not create a clinician–patient relationship, and it is not a substitute for an evaluation by a licensed clinician who knows your history. Always talk with your physician or physical therapist before changing your activity, exercise, or medications. In an emergency, call 911.
Text message (SMS) terms
If you consent to receive text messages from us, the following terms apply.
Message types. We send patient portal sign-in links and payment confirmations for copays and product purchases. We do not send marketing, promotional, or appointment reminder messages.
Message frequency. Message frequency may vary.
Cost. Standard message and data rates may apply. We do not charge for text messages; any charges come from your mobile carrier.
Opting out and help. Reply STOP at any time to stop receiving messages; you will receive one confirmation and nothing after that. Reply START to resume messages. Reply HELP for help, call (346) 278-6634, or email info@udofiaclinic.com.
Delivery. Mobile carriers are not liable for delayed or undelivered messages.
Consent. Consent is not a condition of care or purchase, and opting out does not affect your treatment. Consent is collected on a signed paper form during a visit, or when you text us first; there is no text message sign-up on this website.
Your mobile information will not be sold or shared with third parties for promotional or marketing purposes.
Products, orders and payment
Product prices and availability are subject to change without notice. We reserve the right to decline or cancel an order — including for availability, pricing errors, or suspected misuse — and will refund any payment collected for a cancelled order. Payment is due at the time of purchase unless other arrangements are made in advance. Products recommended as part of your plan of care may be billed to insurance where coverage applies. Returns and exchanges are accepted for unopened, unused products; custom or fitted items may not be returnable. Ask our staff about the return policy for a specific product.
Appointments, billing and cancellations
Appointment requests made through this website are not confirmed until our staff contacts you. Please provide at least 24 hours’ notice if you need to cancel or reschedule. Repeated late cancellations or missed appointments may result in a cancellation fee. Copays, deductibles, and non-covered services are due at the time of service. As a courtesy, we bill insurance plans we participate with; however, coverage is determined by your plan, and you are responsible for any amounts your plan does not pay.
Intellectual property
All content on this site — including text, graphics, logos, and images — is the property of The Udofia Clinic, PLLC or its licensors and is protected by copyright and other intellectual property laws. You may view and print pages from this site for your personal, non-commercial use. Any other reproduction, distribution, or use of site content requires our prior written permission.
Limitation of liability and governing law
This site is provided “as is” and “as available.” We work to keep information accurate and the site available, but we do not warrant that the site will be uninterrupted or error-free, and we may change or remove content at any time. To the fullest extent permitted by law, The Udofia Clinic, PLLC is not liable for indirect, incidental, or consequential damages arising from your use of this site. These terms are governed by the laws of the State of Texas, without regard to conflict-of-law rules, and any dispute arising from these terms or this site will be brought in the state or federal courts located in Harris County, Texas.
Changes to these terms
We may update these terms from time to time. The version posted on this page is the version in effect, and material changes will be reflected here before they take effect.
