The Udofia Clinic ("we," "us," "our") provides in-home physical therapy services. This policy explains what information we collect, how we use it, and the choices you have — including around text message communication.

We use your information to schedule and deliver physical therapy visits, document and bill for care, communicate with you about your treatment, and meet our legal and regulatory obligations as a healthcare provider.
Health information (PHI). As a healthcare provider, our handling of your health information is governed by HIPAA. We do not sell your health information, and we share it only as needed to provide your care, receive payment, or as required by law.
If you opt in — either by checking the consent box in our intake system or by signing our paper intake form — we may send you SMS text messages about appointment reminders, billing updates, and other account-related notifications. We do not send clinical details by text; messages link back to our secure patient portal instead.
Health records and communications are stored using encryption at rest, and access to patient records is limited to authorized clinical and administrative staff.
You can withdraw SMS consent at any time by replying STOP to any text from us, or by contacting us directly. Withdrawing consent does not affect your ability to receive care — it only stops text message communication.
