Consent disclosure
How consent is collected
Consent is collected in one of two ways: on a paper form signed in person during a visit, or when a patient texts us first. There is no text message sign-up on this website.
You can view the paper form we use at our SMS consent form.
Confirmation message
After consent is recorded, we send a single confirmation message:
Opting out and getting help
Reply STOP to any message to opt out. You will receive one confirmation that you have been unsubscribed, and no further messages after that. Reply HELP for help, and you will receive our contact information. You can also opt out by calling (346) 278-6634.
Consent is not a condition of care or purchase, and opting out does not affect your treatment.
Message frequency and cost
Message frequency may vary. Standard message and data rates may apply. We do not charge for text messages; any charges come from your mobile carrier.
Privacy of your mobile information
Your mobile information will not be sold or shared with third parties for promotional or marketing purposes. See our Privacy Policy for full details.
