How The Udofia Clinic, PLLC handles your information, including mobile phone numbers and text messaging consent.
Your mobile information
Your mobile information will not be sold or shared with third parties for promotional or marketing purposes. We do not sell mobile phone numbers, SMS opt-in data, or consent records to anyone, for any purpose.
All of the data categories described in this policy exclude text messaging originator opt-in data and consent; this information will not be shared with any third party.
We will not share your opt-in to an SMS campaign with any third party for purposes unrelated to providing you with the services of that campaign. We may share your personal data, including your SMS opt-in or consent status, with third parties that help us provide our messaging services, including but not limited to platform providers, phone companies, and other vendors who assist us in the delivery of text messages. These vendors may use the information only to deliver our messages and may not sell it or use it for their own marketing.
The messages we send are limited to patient portal sign-in links and payment confirmations for copays and product purchases. We do not send marketing, promotional, or appointment reminder messages. Message frequency may vary. Standard message and data rates may apply. Reply STOP to opt out at any time; reply HELP for help. Consent is not a condition of care or purchase, and opting out does not affect your treatment.
Consent is collected on a signed paper form during your visit, or when you text us first. There is no SMS sign-up form on this website.
Information we collect
We collect information that identifies you or relates to your care, including:
- Contact and identification information you provide when you schedule, register, or contact us, such as your name, phone number, email address, date of birth, and insurance information.
- Health information you share through intake forms, the patient portal, text messages, or during your treatment.
- Technical information collected automatically when you browse this site, such as your device type, browser, and pages visited.
Text messaging opt-in data and consent records are excluded from these categories and are handled as described in “Your mobile information” above. We do not sell mobile phone numbers, SMS opt-in data, or consent records to anyone, for any purpose.
How we use your information
We use your information to:
- Schedule appointments, provide treatment, and manage your care.
- Bill you, your insurance plan, or other responsible parties for services and products.
- Send patient portal sign-in links and payment confirmations by text message, only with your consent.
- Respond to your questions and requests.
- Maintain records as required by law and professional standards.
We do not use or disclose your information for third-party marketing.
How we share your information
We may share your information only:
- With service providers and partners who help us operate our practice — such as our electronic records, billing, and messaging vendors — under agreements requiring them to protect it and use it only on our behalf.
- With insurance companies and other payers to process claims for your care.
- When disclosure is required by law, regulation, or legal process.
We do not sell your information. As stated above, text messaging originator opt-in data and consent will not be shared with any third party for purposes unrelated to providing you with the services of that campaign.
Health information and HIPAA
Information about your health that identifies you — “protected health information” (PHI) — is governed by the Health Insurance Portability and Accountability Act (HIPAA) and by our Notice of Privacy Practices. That Notice describes how your health information is used and disclosed, your rights (including the right to access your records and request corrections), and how to file a complaint with the U.S. Department of Health and Human Services. This website policy supplements, and does not replace, our Notice of Privacy Practices. Questions about your health information may be directed to us using the contact information below.
Data retention and security
We retain patient and visitor information only as long as needed for treatment, billing, legal compliance, and recordkeeping requirements under Texas law and professional standards. We protect information with administrative, technical, and physical safeguards, including access controls, encryption where appropriate, staff training, and written agreements with vendors that handle information for us. No method of transmission or storage is completely secure, and we cannot guarantee absolute security.
Your choices
You may ask to review, update, or correct your personal information at any time by contacting us. You may stop text messages at any time by replying STOP to any message or by contacting us directly; reply HELP for assistance. Consent to text messages is not a condition of receiving care or purchasing products, and opting out will not affect your treatment.
